Complete an SBAR on one of your patients. Use your course textbook and other course materials to review the concepts of Social Determinants of Health (SDoH), Cultural Assessment, and Health Literacy. Visit the CDC website to learn more about SDoH, cultural awareness in health care, and the importance of understanding patient’s health literacy needs. Then, select one area of need for further education, or possibly re-education for your patient. Answer the questions below in the teaching plan section. Submit your SBAR and Teaching Plan to your Clinical Instructor for feedback by their indicated deadline. Make corrections or answer further questions based on their feedback and submit your final work by the end of week 7 to your Canvas course Drop Box.

In this clinical teaching plan, the patient is a child who is coming to the clinic for vaccinations required for 7th grade. The vaccines include tdap, meningitis booster, and an optional HPV vaccine. The goal is to provide education to both the mother and the child about the vaccines, their importance, and any potential concerns or questions they may have. The teaching plan will address the social determinants of health, cultural needs, and the patient’s health literacy to ensure effective learning and understanding.
1. What will you be teaching this patient and why?
The teaching will focus on providing information about the tdap, meningitis, and HPV vaccines. The purpose is to educate the patient and their mother about the importance of these vaccines in preventing diseases and keeping the child healthy. This information will help them make informed decisions about receiving the recommended vaccines.
2. What is your expected outcome following the teaching session? What is your SMART goal?
The expected outcome is for the patient and their mother to have a clear understanding of the benefits and risks associated with the recommended vaccines. The SMART goal is for 100% of the taught individuals to be able to accurately describe the purpose, importance, and potential side effects of each vaccine.
3. What social determinants of health impact your education of this patient? How will you adjust your educational information to accommodate this need?
Social determinants of health that may impact the education of this patient include access to healthcare, socioeconomic status, and cultural beliefs. To accommodate these needs, the educational information will be provided in a culturally sensitive manner and use language that is easy to understand. Additionally, resources regarding vaccine access and financial assistance programs will be shared, if applicable.
4. What cultural needs exist, and how does your teaching plan meet the cultural needs of your patient?
Cultural needs may include beliefs, values, and traditions related to vaccinations. The teaching plan will respect cultural beliefs and offer a culturally sensitive approach. For example, the teaching plan may address common misconceptions or concerns specific to the patient’s cultural background, providing evidence-based information to address any doubts and build trust.
5. What is your patient’s health literacy and ability to understand information being given? Are there potential barriers to learning? How will you accommodate your patient, and what type of learning materials will be provided to help them learn and retain the information provided?
Assessing the patient’s health literacy is crucial to ensure effective communication. Simple and clear language will be used during the teaching session, avoiding medical jargon. Visual aids, such as diagrams or videos, can be provided to enhance understanding. Additionally, a feedback mechanism will be established to address any questions or concerns that may arise after the teaching session, ensuring ongoing support and guidance.
